I hereby certify that I am in good health and have trained for/am physically capable of completing the distance of the events I am entering. I recognize and acknowledge that there are certain risks of physical injury to participants in the events/programs/activities in which I am participating, and I voluntarily agree to assume all risks associated with the participation of the events including, but not limited to: falls, contact with other participants, contact with dogs, the effects of the weather, traffic and road conditions. I also acknowledge the presence of animals in the activities on the premises of the events in which I am participating. Having read and acknowledged the risks involved with my participation, I hereby waive and release: the Glen Ellyn Park District, the Village of Glen Ellyn, the County of DuPage, and USATF, from all claims or liabilities of any kind arising out of my participation in this event even though that liability may arise out of negligence or carelessness in the part of the persons named in this waiver. I permit the taking of photographs, audio, or videotaping during the Woofin & Hoofin activities for publication and use as Woofin & Hoofin deems appropriate. I hereby certify that my dog is in good health and has trained/ is physically capable of completing the

I also certify that my dog has received any and all vaccinations required by his/her veterinarian and has received a physical check up by his/her veterinarian within the past 12 months. I furthermore acknowledge the fact that other dogs will be participating in these events and assume all risks associated. I also assume responsibility for the behavior of my dog and certify that I will take all precautions to ensure the safety of both my dog and others with whom s/he may come in contact.
